Use this form to upload copies of receipts for EMS approved expenses on your EMS company credit card. 

For specific projects, ensure a Daily Field Report is filled out accurately and in a timely manner. 

For projects that require a JHA, complete this form prior to beginning work or new activities. 

All picking tickets must be uploaded through the Picking Ticket Upload tool. Physical copies can be turned into the office, but only after you have uploaded the documents here. 

When working on a scaffold, ensure that the scaffold is inspected by a Competent Person and a full scaffold inspection is complete prior to work beginning. 

This report is for APA064 only. Complete this form when performing hot work and pin welding on site. Once complete, the form will automatically be sent to Whiting Turner for their review and approval.
